IRVINE, Calif. (August 25, 2020) – Pinnacle Claims Management, Inc., in partnership with Pinnacle Health Management, has announced the launch of its Enhanced Diabetes Program. The program will enable eligible participants to leverage advanced mobile technology to manage their diabetes more effectively.
Participants of the program have the ability to monitor their glucose readings through a Bluetooth-enabled blood glucose monitor. This device automatically connects with Pinnacle Health Management’s mobile application “PHM Your Numbers” and stores the participant’s blood glucose readings. The mobile application syncs with a portal and shares the data with the patient’s nurse care manager. The data is also shareable with the patient’s primary care physician.

It’s important to give them a convenient way to record and share their blood sugar readings with their health care professionals to minimize potential risk.”
Participants of the Enhanced Diabetes Program receive a blood glucose monitor and diabetic supplies (lancets, lance pen, and test strips), at no cost to the participant. Pinnacle also sends refills to participants every 90 days. The “PHM Your Numbers” app is available to members in English and Spanish. (Download the app in the Apple Store here.
